•    WHAT IS FAMILY MEDICINE?

Family medicine is a medical specialty devoted to comprehensive health care for people of all ages. This division of primary care provides continuing and comprehensive health care for the individual and family across all ages, sexes, diseases, and parts of the body. •    APPOINTMENTS

-Appointments are generally scheduled two weeks in advance,
however sick appointments are available on a daily basis.
-To schedule an appointment, call (708) 479-8889.

•    VISIT PREPARATION

-Please bring your current prescription bottles (with labels).
-Optional: Bring any expired medications for safe disposal.
-Optional: Write down a problem list/reasons for your visit.
This can help the doctor maximize his time with each patient.

•    NEW PATIENTS

-Dr. Schiappa maintains an open practice; new patients are always welcome.

-We ask that all new patients bring the following to your first appointment:
-Driver’s license/state id
-Current insurance card
-Current prescription bottles (with labels)
-Medical history list/chart (optional)
-Problem list/reasons for your visit (optional)
-Patients under 18 yrs must be accompanied by a parent / legal guardian.

-We also ask all new patients to complete the forms below.

•    HOSPITALIZATION

-Dr. Schiappa visits his patients daily at Palos Community Hospital.
-Upon discharge, Dr. Schiappa suggests a follow-up appointment at his office.
